To apply to us all you need to do is complete the application form you can download it by clicking the button below.

On the application you’ll need to explain what your needs are and a little bit about yourself, then ask your Social Care or Healthcare Professional to complete a short reference about your caring situation and submit your application. We treat every application with compassion and understanding.

Some applications may require that a member of the Leonardo Team will ask to visit you at a convenient time to meet you to discuss your situation as there may be some other services and support we can offer you.

If you are referring a client for the counselling service and you are a professional employed by BCP Council or Dorset Council, please contact the office for the relevant forms.
